Structuring a lesson is challenging, especially as it requires revision and alteration throughout not only a term but often one's entire career (especially early on). In particular, once the core activities have been determined to work with the target language and tangible elements of the class (number, language level, teaching aids, etc.), it can be hard to iron out elements such as time and feasibility of the learner objectives within the allotted time, and can require a lot of editing over time.
As a relatively new but also somewhat experienced teacher, the above issues certainly are present for me. In particular, it can be hard to consider how to structure activities *within* a phase of the lesson, and knowing what constitutes a good order for Study or the difference between an appropriate Engage and Activate phase are still quite hard for me to distinguish other than that Activate uses language learned that day, while Engage doesn't exactly have to even have a relationship to it as long as it is relatively unchallenging and gets students to talk and think.
The sample lessons and structures provided help a lot though, so I'm quite grateful for their inclusion. Creating lessons from scratch is something that can be hard to nail down, but as it says in the materials here, one shouldn't worry about it being set in stone.
