What I've experienced as a student is that when it comes to writing teachers will always ask you to create stories, that's the most common and useful excercise, of course they'll always try to prepare the activity making you think about some vocabulary first, then while you're writing, teachers will help you with grammar structures.
I strongly believe that writing is a skill developed thanks to reading. If there is no reading there won't be a good vocabulary and structure comprehension when writing.
About speaking, I think that making the difference between fluency and accuracy is a great way to make students aware of what we want from them regarding different activities and their levels.
